Turnout.co is a venture-backed advocacy company that helps people get the benefits and care they're entitled to. We work across Veterans Benefits, Social Security, Education Advocacy, and Medicare. Our Healthcare Advocacy (HA) team pairs Medicare patients with trained advocates. The advocates help patients living with serious illness coordinate their care, resolve coverage and access problems, and connect with community resources.

The Role
You'll be a supervising and billing physician for the HA program. You'll conduct the telehealth visits that establish each patient's need for navigation services, set the plan our advocates carry out, and provide clinical oversight while they work. You won't be managing patients' primary care. Your role is the clinical foundation that makes our advocacy safe, effective, and compliant.

What You'll Do

  • Conduct telehealth initiating visits with Medicare patients. You'll assess serious or high-risk conditions and determine medical necessity for Principal Illness Navigation (PIN) and Community Health Integration (CHI) services.
  • Document each patient's condition, goals, and navigation plan in the EHR to meet Medicare requirements.
  • Provide general supervision to a team of non-clinical healthcare advocates. This means reviewing cases, answering clinical questions, and escalating patients who need a higher level of care.
  • Review and sign off on advocate service documentation billed under your NPI.
  • Coordinate with patients' treating physicians and specialists when needed.
  • Maintain licensure, Medicare enrollment, and payer credentialing across your states. Our team supports you on this.
  • Stay current on CMS rules for PIN, CHI, telehealth, and supervision, and help keep the program compliant.
  • Contribute to advocate training, clinical protocols, and quality standards.

Requirements

  • MD or DO, board certified in internal medicine, family medicine, geriatrics, or a related specialty
  • Active, unrestricted medical licenses in multiple states (required); 10+ states strongly preferred. Interstate Medical Licensure Compact (IMLC) eligibility is a plus.
  • Active Medicare enrollment, or eligibility to enroll, with no sanctions or exclusions
  • 3+ years of clinical experience with older adults or patients with chronic or serious illness
  • Telehealth experience
  • Working knowledge of Medicare documentation and billing; experience with PIN, CHI, or Chronic Care Management (CCM) is a plus
  • Comfortable supervising non-clinical staff in a fast-moving startup environment
