Base pay range: $59,300.00/yr - $80,900.00/yr
Responsibilities
- Apply clinical knowledge to educate and empower members to take charge of their own healthcare decisions.
- Provide guidance and clinical expertise on how to navigate through the health care setting.
- Provide ongoing consultation with members via telephone to ensure that they are receiving the best quality of care possible.
- Guide members and their families toward and facilitate interaction with resources appropriate for their care and wellbeing.
- Coordinate community care and services as deemed appropriate.
- Proactively outreach telephonically to eligible Humana members and engage participation in appropriate care management programs.
- Complete physical, psychological, emotional and environmental assessments, for the purpose of providing appropriate, timely interventions to ensure provision of optimal care.
- Based on identified needs, recommend and/or coordinate interventions that may include information, education, resources and referrals.
- Work collaboratively with other members of the Interdisciplinary team including LAL, IHWA, Social Services, Pharmacists, Psychiatrists, Utilization Management teams, Community Health Educators, EAP and others if appropriate.
- Comply with performance and reporting standards as defined by Humana Corporation.
- Utilize motivational interviewing and solution-oriented approaches in communication with members.
Qualifications
Required Qualifications
- Master\'s degree in a behavioral-health field, such as social work, psychology, or related health discipline from an accredited university.
- Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) in the state of Illinois or ability to obtain IL licensure.
- Minimum of 3 years of experience working as a licensed Social Worker.
Preferred Qualifications
- CCM certification.
- Managed care experience.
- Bilingual in English/Spanish.
- Experience working with the geriatric population.
- Knowledge of community health and social service agencies and additional community resources.
Additional Information
- Workstyle: remote, work from home.
- Work Location: Illinois preferred.
- Typical work days/hours: Monday – Friday, 8:00am – 5:00pm CST, occasional flexibility in scheduling may be required to accommodate member needs.
- Travel: Occasional travel to Humana\'s offices for training or meetings may be required.
Internet and Equipment
- To ensure home or hybrid home/office employees’ ability to work effectively, internet service must meet criteria of minimum speeds (25 Mbps download, 10 Mbps upload).
- Humana will provide home or hybrid employees with telephone equipment appropriate to meet business requirements.
- Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.

Travel: While this is a remote position, occasional travel to Humana\'s offices for training or meetings may be required.
